I need a referral from the DVM before I can complete an evaluation on your dog. Humans, I can treat you for 30 days before needing a referral.
I deeply value my partnerships with referring veterinarians. Submitting a completed referral form from the DVM is imperative for each patient before I can provide an initial consultation. This collaborative approach ensures a seamless and comprehensive care experience while maintaining trust-based relationships with DVM’s and specialists. If additional care or diagnostics are needed, I will always refer patients back to their primary veterinarian or the appropriate specialist. Humans, I can treat you for 30 days before requiring a referral. You will then need to see your PCP or specialist for a consultation to see if additional testing or imaging is required. This is a state of Florida requirement.
Effective communication between providers is essential to the health and well-being of every patient. That’s why we promptly share all initial consultation findings and progress reports with the patient’s primary care provider and DVM, fostering a seamless continuum of care.
I appreciate you taking the time to complete and submit this referral form—your collaboration makes a meaningful difference.
